What is Community Nursing?
Community nursing under the NDIS is a specialised service provided by registered nurses to participants with special needs – either in their own home or out in the community (shared homes or SDA facilities). The goal is to promote independence and comfort to ailing participants needing wound care, chronic disease management, continence support and more.
Based on your NDIS goals and preferences, Care and Beyond will tailor the services to provide you or your loved ones with safe, respectful care to address the unique needs of the participants. This allows the participants to avoid frequent hospital visits or needing frequent emergency interventions.

- Wound Care Management: From assessing wounds to dressing them, we also manage chronic wounds.
- Chronic Disease Management: We provide nursing care for participants with diabetes, cardiovascular diseases, respiratory diseases or other health conditions.
- Medication Management: From medication reminders to administration, we also help with injection administration.
- Continence Management: Our nurses are trained in stoma and catheter care in a respectful and dignified way.
- Post-Hospital Support: From monitoring your vital signs to creating a report on your health, we collaborate with GPs to provide an all-around treatment.
